Please complete this form to have you transcript sent to the college of your choice. The first TEN (10) transcripts will be at NO CHARGE while enrolled at LCA. After 10 transcripts have been sent, OR after graduation, transcripts will be sent for a charge of $1.00 per transcript. *If you are a current student at LCA please call first before filling out this form.
